In preparation for next year I am attaching the enrollment application for Carthay's STAR free after school program for next year (2024-2025) to this email!
STEP ONE:
Print and fill out the desired application!
Please complete only one form per student and choose whichever language is preferred.
(To save paper and prevent overprinting, limited copies will be available in Carthay's front office. Additional copies will be made upon request. To request a printed copy, please get in touch with me, and I will gladly accommodate!) Applications will be available throughout next year as well. Enrollment is set to be open all year, but students must complete an application to participate. ALL STUDENTS, INCLUDING RETURNING/PREVIOUS STAR STUDENTS, MUST HAVE A NEW 2024-2025 SCHOOL YEAR APPLICATION FILLED OUT!
STEP TWO:
Please deliver the application to the Carthay front desk or to a STAR teacher upon pickup after school. DO NOT SEND THE APPLICATION WITH A STUDENT.
